1. Pre-Screening Set Up Process:

Create TWO identical visual screening lines side by side by placing the Tumbling E’s Eye charts on walls or solid surfaces at eye level (persons being tested may be seated or stand). The charts should be in well-lit areas (including natural lighting).

  1. Using the provided tape measure, mark 20 feet (6 meters) from the wall. The charts should be in well-lit areas (including natural lighting).
  2. On the Measurement/Record-Keeping Form, write the date of the screening, location of the screening, the name of the person conducting the testing.
  3. Write the name of the person being tested and his age on the Measurement/Record-Keeping Form
  4. Please note that the tester/examiner should hold the First Sight lens tree while determining the appropriate lens strength needed for the recipient’s eyeglasses. The recipient should not hold the lens tree.

2. Screening Process & Instructions for Myopia – Near Sight

STEP 1:  Have the Person being tested cover/occlude the left eye with the Eyesight Blocker (included in the First Sight Kit).

STEP 2: Have the person being tested read the Es on the Tumbling Eyechart by telling you what direction the E is facing (a flat hand gesture indicates the direction of the E).

STEP 3: Start at the top and move down the chart.

STEP 4: Determine the point at which the person being tested can no longer clearly identify the direction of the E on the chart.

STEP 5: The last line they read correctly is their uncorrected visual acuity (UCVA).

STEP 6: Repeat the above procedure for the right eye.

STEP 7: If the person being tested can read all the way to the bottom of the eye chart, his/her eyesight is normal.

STEP 8: If the person being tested can’t read all the way to the bottom of the eye chart, then move the person being tested to the next identical screening station and follow Steps 9 -13.

STEP 9: With the left eye occluded again with the eye blocker.

STEP 10: Ask the person being tested to look at the lowest line so that they can read without any difficulty.

STEP 11: Using the Screening Lens Bar/Lens Tree over the right eye (left should remain occluded) – increase power by beginning with “0” lens and then moving it to -1.00, -1.5, -2.0 and gradually increasing to the next lens strength level, until the person being tested reads the 20/20 line or until they can’t read any further (i.e., can’t read below the 20/60 line). With each step, ask the person being tested if they can see the Chart lines clearly or which lens feels better.

STEP 12: Use a similar procedure for the right eye. Again, with each step, ask the person being tested if they can see the Chart lines clearly or which lens feels better.

STEP 13: When they choose which lens strength works best for them, write down this measurement in Measurement/Record-Keeping Form for the person being tested.

STEP 14: Once the appropriate strengths of the lenses are properly identified (as noted on the Data Form) for that person, then: (i) take a blank frame from the First Sight frames box and (ii) pick the two appropriate lenses identified during the screening process from the First Sight Lens box. Each lens’ strength is clearly marked on the lens envelopes. Important Note: Please make sure the correct lens strengths are placed in the corresponding Left and Right sides of the frame. It’s sometimes easy to make a mistake, so please be very careful about this.

STEP 15: Place/Snap-in the appropriate lenses in blank frame. Depending on the room temperature, you may have to apply some pressure on the lens to make it fit properly in the frame. Or you may use the supplied Lens Application Tool/Pliers. Click Here

STEP 16: Clean and adjust the eyeglasses for comfort and clarity. Make sure the eyeglasses fit properly and get the desired result. Use self-adhesive pads for a comfortable fit.

STEP 17: Once the recipients have their new pair of First Sight eyeglasses, have them go once again in front of the Eye Chart (6 meters/20 ft away), blocking each eye, going again through the E eye chart to make sure that the lenses received by the recipients are correct.

STEP 18: Place the customized pair of eyeglasses in the provided First Sight pouch/cleaner and deliver it to the person being tested.

STEP 19: Advise the person receiving a new pair of eyeglasses to handle the glasses with great care in order to keep the lenses and frame from being scratched or broken.

STEP 20: It is highly recommended to wipe the lenses on the Lens Tree after each individual screening process with medical grade wipes to avoid cross-contamination.

STEP 21: The prescription of the person being tested will naturally change over a period, so please arrange to have the above test repeated, preferably, annually.

STEP 22: At the end of the completion of the screening and delivery process, please gather all the Data Forms and send them to First Sight by email to: nmamdani@firstsighteyeglasses.com for record keeping purposes.

STEP 23: Before you start screening the next recipient, please clean the lenses on the lens tree and the eyesight blocker used to cover the eye.

Recycle: First Sight eyeglasses are Recyclable. If the prescription of the recipient of the eyeglass changes, have the person return the eyeglass and give him a new pair. Then just remove the old lenses from the replaced frame, clean and make it ready for another recipient.

Please note that if you intend to use our FirstSightKit® in the United States of America or Canada, you must have authorized personnel for visual screening of the recipient. Once the vision screening prescription has been certified by an authorized person, First Sight eyeglasses can be used to provide the recipient with the properly fitted prescription lenses and glasses.

This may not be applicable to the First Sight eyeglasses used in other countries. First Sight highly recommends contacting local authorities to ensure compliance
